How do I enroll in HeadsUp! Reading?

Enrollment varies by state. Check below for the states that are participating on a state-wide basis and contact the person indicated if your state is listed. If your state is not listed:

• Determine whether your site subscribes to the HeadsUp! Network.
• If not, download, fill out and fax or mail the subscriber agreement to the address indicated on the agreement here, to subscribe and get the equipment installed.
• Identify a facilitator at your site.
  Fax their name, title, phone number e-mail and mailing address to (703) 739-0878.
• Click below to receive the facilitator packet.
• Then, enroll with your facilitator.
